What is the smell coming from my wisdom tooth?

January 14th, 2013
Q.

I have a wisdom tooth that has a huge cavity in it, the nerve probably died a while ago as I experience no pain whatsoever when cold or very hot stuff touches the tooth. About a month ago however I had a huge pain that was redirected in my ear, I went to the doctor convinced I had otitis and she told me its actually the cavity in my mouth that was causing the pain. After that I had no further pain.

Yesterday I noticed a funny, unpleasant smell and taste coming from that tooth. Brushing my teeth doesn’t make it go away for long. I am very scared of the prospect of this situation building up into a need to go to the emergency where I can’t even chose my doctor.

As you can imagine I am petrified. I have the worst kind of dental phobia, all my hair stands up just thinking about it. In fact I am so scared that I was unable to eat since I realized the smell, I completely lost my appetite and I can’t focus on work.

Please tell me, what could it be ? Infection ? What are the possible outcomes and what can I do about it to prevent an emergency type situation ?

Can you advise me about oral sedation?

January 11th, 2013
Q.

Hello Dr Pinder, I hope you don’t mind me messaging you but after spending hours online looking for advice, I came across this site and wondered if you could help? I don’t really know where to start…
When I was in my teens, I read an article somewhere that said that people who suffer with M.E can react to silver fillings. Suffering with M.E myself, I discussed this with my dentist at the time, who agreed to give me a white filling. Unfortunately for me, he carried out my filling without giving me an injection and as a result of this, he drilled too low and I’m guessing, caught my nerve…he even carried on when I screamed out and started crying. Until that point, I had never been scared of the dentist but because of what happened, I never went back to a dentist for about 10 years and only then because I was in extreme pain. Luckily, I found a really nice dentist who is really patient with me but even now, all I can manage is a check up. He referred me to a surgery that carries iv sedation and I was so happy knowing that I wouldn’t have a clue what was being done, I jumped at it. The first lot of sedation went really well, I woke up in the recovery room not remembering anything but unfortunately, the staff were unhappy that i had taken a long time to wake up. My mum explained to them about my M.E and it was normal for me not to wake up even without being medicated but still they weren’t happy. Since that time, I have had to go back for sedation several times for further work as my teeth break really easily but my happy, relaxed experience has been replaced with worry and anxiety. I started to become ‘aware’ of what was happening to me during further visits. I could hear the dentists talking to each other and hearing the drill (which really scares me.) In one sedation session, I was told to stop crying and calm down. Since then, I have come out of every session crying. I have had three different adults accompany me to the clinic and they have all said that the staff have been really impatient with me crying. On my last visit, they told my friend that the Government have changed their rules and they have given me the maximum dose of sedation they can give and if I continue to be that upset, then perhaps I should go to the hospital for any more treatment. To be honest I don’t think this is really fair as, in a consultation I once had there, I apologised to a different dentist there who assured me that it didn’t matter that I got upset, he explained that some people have no reaction to sedation, that some people get angry and some people cry but not to worry.
In December 2012 had two sessions there, the first was for root canal treatment and the second, I thought was for a crown to be fitted but when I got home and looked, all he had done was filled what was left of my half tooth. Part of my tooth has now chipped off, I have rung my dentist and he said that I have to contact the clinic myself to see if they will fit a crown on the NHS or if I will have to pay for it. I am worried about this as I currently am receiving Employment and Support Allowance and they haven’t charged me before but what really is worrying me is having to have sedation again and that they may be impatient with me again. They are the only clinic who deals with sedation in my area and my local hospital only deals with extractions so unless I can have the work carried out with my normal dentist, I have no choice but to go back. I was wondering if you could give me some advice regarding sedation please? I have looked online to see about sedation doses but all I can see is that normally one drug is given in a titrated dose but in some cases a second drug may be used. I couldn’t see anything about a maximum dose. I just don’t understand why the sedation sessions have changed so much. I hope you don’t mind me asking but I really need some advice as don’t have anyone I can turn to. Thank you for taking the time to read this. Yours sincerely.

How can I overcome my phobia of the dentist for my daughter’s wedding?

January 9th, 2013
Q.

Hello Dr,
I am 67 years old and very fearful of going to the dentist. I had bad experiences as a child and in my early adulthood which put me off for life.I have never been that bothered and have been quite lucky that I have never had awful pain or anything. On the other hand my teeth look awful and my daughter has taken me to one side and asked me to get them sorted out for her wedding. Part of me wants to make her happy but a larger part of me knows I won’t be able to go through with it. I am not sure what to do and wondered if you could advise me at all?

How can I get over my fear of needles so that I can seek treatment?

January 7th, 2013
Q.

Hello, I cracked my bottom 3 from last tooth about 6 months ago, I went to the dentist and had a tempory filling but he noted I would have to come back as the same tooth on the bottom right was also ‘going’. the bottom right tooth has now also gone so the same tooth on both sides is now cracked in half.

I dont have a problem with dentists but i do have a problem with needles, especially needles in the mouth. I’ve never had one and avoided them at all costs.

I currently don’t have any pain with my cracked teeth, do you think they will be able to just fill them or will they have to remove them?

Also I use an NHS dentist, do they offer any other way of anesthetic other than the needles?

Thanks

How can I get treatment when I am too scared to return to the dentist?

January 3rd, 2013
Q.

I visited a dentist a year ago and was diagnosed with 3 teeth suffering infection, extraction was offered. My fear was oppressive after 4 wisdom teeth extractions failed in the chair years earlier resulting in one cracking in half, successive failings to numb my teeth during many visits for fillings, having 4 teeth extracted as a child due to having too many teeth allegedly?

For my wisdom teeth I was eventually offered GA and found it peaceful, but this dentist tried to numb the first of 3 extractions and failed, the pain was intense and thankfully she stopped and decided to drill and temporarily fill them instead and refer me to a specialist…. the treatment was hell, the infected roots prevented full numbing but somehow I endured.

Then 8 months ago the “specialist” contacted me and I asked how they were to numb me where she had failed…their response was that they were 5 times faster than other dentists, I freaked out, I didn’t want faster I needed painless.

Needless to say I didn’t go, I now have an abscess in one of the teeth, but shame and utter fear prevent me seeing a dentist again. Can I see a GP and ask for referral for GA? Otherwise I am resigned to accepting the pain, praying they fall out on their own and risking my life I guess with the abscess infection spreading.

How can I get over my dental phobia and have my teeth restored?

January 3rd, 2013
Q.

Hello, I have suffered a long time with anxiety/depression/social phobia following a head injury back in 1990 ( RTA ).
I have a dentist phobia following a particularly nasty experience with not having been numbed but the dentist carried on drilling regardless saying I was imagining it etc. Fast forward 10-15 years, and I realize a lot of my social phobia is down to having awful teeth. I do not have an NHS dentist as they’re quite a rarity where I live, the last one taking patients on was 70 miles away until recently, and the one currently taking NHS patients has quite a lot of poor reviews from people which is putting me of going to see them for anything at all.
December last year I was in a car accident ( passenger ) and I lost a front incisor, plus broke a front incisor crown. I made a temp acrylic crown for my broken tooth and fixed it in with glass ionomer purchased from ebay. I repaired as best I could the broken crown with the same material. I did this as the thought of visiting an emergency dentist would mean extractions and no front teeth, with no dentist for follow up treatment, which in my circumstances would leave me suicidal. The thought of extractions horrify me but not as much as the thought of having a big fat denture gagging me and covering my upper palate. I have little to no self confidence as it is, but the thought of a denture is ten times worse than my current situation. There’s absolutely no way I can afford private treatment as I’m not working. Is there any place such as a dental school etc I could get implants and crowns done at NHS prices if I volunteer myself as a live ‘dummy’ ‘test bed’ for the students to work on ?
A healthy nice smile would give me the confidence to possibly start interacting with society again ( I’ve pretty much shut myself away for 22 years, avoiding social interaction and I have remained single ) It seems I’m written off, and any time I think of turning myself around I’m met with a wall. This time, more than any fear of a dentist, it’s the cost, and the limits. It’s a vicious circle. I want to get my confidence back and become a happy confident useful productive member of society again, yet I can’t as the very fear and cost of dental treatment negates the possibility of turning myself around.

Why did I have extreme sickness after my procedure?

December 18th, 2012
Q.

I have advanced periodontal disease and had not been to the dentist for 10 years, eventually I went after realising 6 of my teeth were loose, the dentist I went to wanted to perform a deep clean top and bottom, I had explained about my phobia of the dentist and my anxiety, he was unsympathetic and on my second visit to him had an allergic reaction to the anaesthetic causing severe dizziness, vomiting and swelling, he told me it was my fault for being to sensitive carried on with the procedure and then left me on my own vomiting in a public place whilst propped up against a wall. Please can you tell me what the correct procedure should be if a patient displays these symptoms, I had had 6 local injections over a period of 2 days, was I perhaps overdosed?

How can I get over my fear and finally get my teeth straightened?

December 13th, 2012
Q.

Hello Dr Pinder, I’m 24 and have been terrified of the dentist since I can remember. The last time I visited the dentist I was 14 and I had been told I needed braces. Upon the day of my brace fitting I completely freaked out in the waiting room and ran out in a panic vowing never to return, which I haven’t. But now my teeth are so crooked I’m embarrassed to smile anymore. It’s really affecting my confidence and self-esteem and I want desperately to have them sorted out but I just can’t face it. Every time I even think about phoning up the dentist I start sweating and hyperventilating. I really want to overcome this fear in order to regain confidence in my smile again, but I just can’t push myself to go to the dentist. Do you have any advice to help me go to the dentist and overcome my fear?I’m desperate. Help me please!!

December 13th, 2012
Q.

I took the first step today by going to a dentist
they gave me some antibiotics which I need to take but when I finish the course of antibiotics I still can’t let the dentist look at my teeth properly so they can clean them. I have got a lot of tartar on them, can I ask the dentist to refer me to the dental hospital to have my teeth out?
